Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.

We’re happy to accept most dental insurance plans. Dr. Hagerty is a preferred provider for Delta Dental. Our front-office team will electronically file your claim and work with your insurance company to expedite processing and maximize benefits.
